How do I submit our club's COVID supervisor's email addresses?
In order for your supervisors to have access to players questionnaires, you must get your club secretary to fill out this Microsoft Form. This form can only be accessed using a GAA/LGFA/Camogie official email account i.e firstname.lastname@example.org.
Please ensure that within the form, your club name is stated clearly including the association which you need access to i.e GAA/ LGFA / Camogie.
This form must be filled out separately if your supervisors need access to more than one association or independent team. For example, if your GAA club has a separate independent team, you will need to submit the Microsoft Form twice: 1 for the Parent Club and 1 for the independent team. (Please ensure that both club names/independent teams are clearly stated within the form).
I am a COVID supervisor however I cannot view our member's health questionnaires?
In order to have COVID supervisor access, you will need to ensure the following:
1. Your club has submitted your email address via this Microsoft Form.
2. You have ticked the 'I am a COVID Supervisor' option at the bottom of your profile on the Return to Play system.
I am a COVID supervisor however I can only see our GAA player questionnaires and not our Camogie player questionnaires?
Please be advised that the Microsoft Form must be submitted separately for each association you wish to have access to. For example, if your club submitted the form for GAA however you still cannot access the LGFA or Camogie questionnaires then you will your club to re-submit the Microsoft Form again.
Please ensure that within the form that the association is clearly stated in the club field i.e GAA, LGFA or Camogie.
I have COVID supervisor access however some questionnaires as missing?
If you have access and notice that you can't see a particular member's questionnaire then this member may have assigned the wrong club to their profile.
Please correspond with your club members which club and association they need to pick. If a member is filling out multiple health questionnaires for their dependants of different associations they must add the GAA and LGFA club to their profile. This ensures the clubs will be available for selection when completing the Health Questionnaire.
What does the amber submission icon mean?
If you or a member has noticed that 'Submitted' appears in amber/orange then the member has not been cleared to play. This means that the member has selected 'Yes' to one or more of the options on the health questionnaire.
The warning text on the health questionnaire explains that 'If you have answered YES to any of these questions you should stay at home and contact your GP by phone for further advice.' Please follow this advice.
Did you or a member submit an amber questionnaire by accident?
If for any reason, you or a member has submitted an amber questionnaire by accident then we would advise that you add a brand new questionnaire to the system and let your COVID supervisor know.
How can I delete or edit my submitted Health Questionnaire?
Once a health questionnaire has been submitted it cannot be edited or deleted by any user. It is the responsibility of the person completing the health questionnaire that all details are correct before submitting the health questionnaire to your COVID Supervisor.
How long does it take for COVID Supervisor to gain access?
When your club has submitted the email addresses of their supervisors, it can take up to 24/48 hours for the permissions to be assigned.
1. The email address which was supplied to us by your club matches the email address which you used to sign up to the Return to Play system.
2. The ‘I am a COVID Supervisor’ option has been ticked at the bottom of your profile on the Return to Play system.
If the email address supplied to us is incorrect, you have not been assigned to the correct club or you require access to additional clubs/associations then please ask your club secretary to fill out the following form again:
You can assign the same COVID supervisor to your GAA/LGFA/Camogie clubs, however you must fill out the form for each association with a list of supervisor emails.
(Please note that the form above has to be accessed using an official GAA/LGFA/Camogie email account).
If you have not ticked the ‘I am a COVID Supervisor’ option, please navigate back to your profile and update this information. We will then be able to assign you supervisor permissions.
If there are any other questions relatin
